We are a Joint-Commission-accredited community health center providing primary medical care and basic dental care to everyone, including those of limited financial means. We follow the patient-centered-medical-home model and offer a range of ancillary services, including healthcare coordination, on-site behavioral health visits, nutritional counseling and assistance with enrollment in the healthcare marketplace. We have a sliding fee scale for the uninsured. Our patients and staff are a multilingual, diverse and dedicated group. We strive to deliver excellent care, to make patients feel welcome and to treat each other well. Our nurses and providers qualify for National Health Scholar and state and federal loan repayment programs. We have six medical office locations throughout the state and also have a mobile health unit serving migrant and seasonal farm workers during the summer.
